You also authorize disclosure of your health care information included in the story related to medical history, diagnosis, treatment or prognosis for use in reprints including newsletters, newspaper, television, websites, internet, or other media. You agree to allow your article to be published and republished or to be used for any other purpose. You understand and agree that your identity may or may not be released. You agree to hold all associated with distribution of this article of any sort, including The Center for Cancer and Blood Disorders, Susan G. Komen for the Cure, and Sundance Associates harmless from any and all liability arising from these activities. Stories may be edited for punctuation, grammar, length, and style prior to publication. 3. Your entry should be 500 words or less. 4. Entries will not be returned. 5. 1. Contest winners in places 1st, 2nd, and 3rd will receive $50 gift certificates to a local restaurant. In addition, the story will be considered for publication in the upcoming "Inspirational Stories" series. This book is given to cancer patients at The Center for Cancer and Blood Disorders and to the public.
